Every exam follows the Federal Motor Carrier Safety Administration (FMCSA) standards required for interstate commercial drivers. Most appointments take about 30 minutes.
Most drivers who run into trouble are not disqualified — they receive a short-term certificate while a condition is documented or controlled. Knowing what examiners look for is the best way to walk out with a full 24-month card.
Readings at or above 140/90 can shorten your certification; 180/110 or higher disqualifies you until it is controlled. Take your medication as prescribed and avoid caffeine and nicotine before the exam.
Insulin-treated diabetes requires FMCSA form MCSA-5870 from your treating clinician. Poorly controlled A1C or recent severe hypoglycemia is a common reason for a short-term card.
High BMI, loud snoring, and daytime fatigue trigger screening. If you use CPAP, bring a 90-day compliance report showing use at least 4 hours a night, 70% of nights.
You need 20/40 corrected in each eye, 70-degree peripheral vision, and the ability to hear a forced whisper at five feet. Bring your glasses, contacts, and hearing aids.
Some controlled substances, sedatives, and narcotics are disqualifying without a letter from your prescriber confirming you can drive safely. Bring the full list, including dosages.
Heart attack, stents, bypass, pacemakers, seizures, or strokes require clearance from your specialist. Not disclosing them is falsification and risks your certification entirely.
A standard Medical Examiner's Certificate is good for up to 24 months. Certificates of 3, 6, or 12 months are issued when blood pressure, diabetes, sleep apnea, or heart conditions need monitoring. Schedule your renewal at least two weeks before expiration so a lapse never puts you off the road.
Under 140/90 supports a full 24-month card. 140–159/90–99 typically earns a one-time 12-month certificate. 160–179/100–109 gives a 3-month window to get controlled. 180/110 or higher is disqualifying until treated. Rest, medication compliance, and skipping caffeine before the exam matter.
Examiners screen for neck size, BMI, snoring, and daytime sleepiness. A diagnosis does not end your career — documented CPAP compliance of at least 4 hours a night on 70% of nights keeps most drivers certified. Bring your download report to the exam.
Our CDL/DOT physical exam is a flat $100, paid at the time of the exam. There are no hidden fees. Additional testing that some drivers require (such as follow-up documentation from a specialist) is not included.

Bring a photo ID or your CDL, a list of all current medications with dosages and prescribing doctor names, your glasses or contacts, hearing aids if you use them, and any specialist paperwork for conditions such as diabetes, sleep apnea, heart disease, or blood pressure. If you use a CPAP, bring your compliance report.
The most common reasons are uncontrolled high blood pressure, uncontrolled blood sugar, untreated or non-compliant sleep apnea, vision or hearing below FMCSA standards, certain medications, and unreported heart or neurological conditions. Many of these result in a short-term certification rather than a disqualification once documentation is provided.
